When I look at the screenshot you uploaded, I see an open workbook called
"QOS Template - Rev 4_0.xls".

Are you saying that you have not opened this workbook?

Where is this workbook stored? If it's in a startup folder, it'll be loaded
automatically and will be a hidden workbook. You'd stop this automatic load
by moving the file somewhere else.
